Hi, I’m Jeff, a firefighter/paramedic and the person behind Emergency Medical Form (EMF).

I’ve seen firsthand how having important medical information readily available can make a difference during an emergency. That’s why I created Emergency Medical Form, to help individuals, families and first responders be better prepared when every second counts.

Our mission is to make emergency medical information easily accessible when it’s needed most.

We’re working to expand this project by developing a mobile app and QR code system that will allow first responders to quickly access important medical information during an emergency.
