A 43-year-old male sustained a rare and severe thoracoabdominal impalement injury while working on a rural property. During the assembly of a perimeter fence, a heavy agricultural implement—a bale spear—dislodged unexpectedly from a loader, falling directly onto the patient.

The metallic projection, weighing approximately 18 kilograms (40 pounds), entered through the patient’s dorsal region and exited anteriorly through the abdomen. Notably, the apparatus remained partially attached to the vehicle at the time of penetration. Emergency responders, with the aid of coworkers, were forced to use an acetylene torch to sever the impaling device from the loader before the patient could be transported.

Upon airlift to a regional trauma center, the patient underwent an emergency laparotomy lasting over four hours. Surgical exploration revealed that—despite the depth and trajectory of the spear—it narrowly missed the spinal cord, abdominal aorta, and ureter, sparing the patient from immediately fatal hemorrhage or organ failure.

Postoperative recovery was unexpectedly swift. The patient was mobilized within days and discharged from the surgical unit just two weeks following the incident. No permanent neurological or urological deficits were reported, and full recovery is anticipated.

Such impalement injuries are typically associated with high mortality. However, survival may be possible when critical structures are avoided, and when surgical intervention is prompt and multidisciplinary.
