The following is a witness statement from an actual case. This is not how to write it:
"To whom it May concern"
The case caption and docket number should be cited.
"I Eliezer XXX want to make the following statement, that I saw Mrs. Anna XXX XXX [Complainant] emotionally affected due to labor abuse and mistreatment on her work area at the XXXX, XXX, XXX [Employer, department, location]."
Complainant is a better witness to her own emotion than this witness who observed her being emotional.
"Abuse" and "mistreatment" must be described concretely. Date is missing. Location is missing. Information of the supposed abuser and harasser is missing.
"There were times when Ms. XXX informed me that her immediate supervisor was harassing her at work and this caused on her fear and insecurity on being able to carry out her work efficiently."
Again, Complainant herself is a better witness to her own experience than a witness who observed her being harassed and being fearful and insecure or not being able to carry out her work efficiently.
Dates are missing, the name of the harasser is missing along with his or her job title.
There is a big difference between being harassed by a supervisor and by a co-worker. So putting the job title after the name is important.
"One day she was called through the intercom while she was working near my area, when she returned back, she told me that she was reprimanded by her supervisor for not being in her work area."
Again, Complainant is a better witness to this incident of receiving the reprimand.
The date, and the supervisor's name and tile are missing.
If the reprimand is written, the written record would be a better evidence than this witness testimony. In that case, this witness add nothing new on the reprimand incident.
"Mrs. XXX was not given the same equal opportunity as a male employee to be certified on the power motor equipment."
This witness statement is vague and general. It does not prove anything because it lacks the identity of "a male employee" and his job title and to whom he reports to.
If this "male employee" is broached as a comparison against whom Complainant compares herself, it is important that he and she reported to the same supervisor. Otherwise, it may be difficult to argue that he is a proper comparison.
"Any other question please do not hesitate to contact me."
The contact information is missing. A witness should be willing to provide his or her home address, email, and phone number, so that the opposing party's attorney may contact him or her to "cross examine" the testimony. A witness who is not willing to be cross examined is not as credible as the one who is.
The ending with the following "declaration" and signature is missing:

A statement without declaration is counted but is deemed not as credible as one with declaration.
