Please download and complete the attached forms together with your child. There is a parent form for you to fill out and a Friendship Link page for your child to complete in whatever way works best for them.

Once finished, send the completed forms back to us by email or mail so we can add your child’s link to Ben’s growing Friendship Circle.

Every child communicates and shares in their own way, and there is no right or wrong way to complete the page. The goal is simply to share a little about who they are, what they enjoy, and what makes them special.
